Physical Therapy and Occupational Therapy Alumni Achievement Award Nomination Package

The year 2020 marks the 24th anniversary of the Physical Therapy and Occupational Therapy Alumni Achievement Awards. The PT and OT Alumni Achievement Awards, first awarded in June 1996, acknowledge graduates who have made exceptional contributions as members of the Physical Therapy or Occupational Therapy professions. Alumni or members of the public may nominate a University of Toronto graduate in Physical Therapy or Occupa­tional Therapy for this award.

Each year, one alumnus from Physical Therapy and one alumnus from Occupational Therapy receive the award at the Alumni Reunion held in June.

We encourage all alumni or members of the public to consider nominating University of Toronto gradu­ates in Physical Therapy and/or Occupational Therapy for these distinguished awards. The achievements of the alumnus should demonstrate qualitative impact for the health and well-being of individuals, organizations and/or society. The nominations must include evidence of outstanding achievements and contributions to the professions in three or more of the following six categories:

  1. Scholarly activity in clinical, community, or academic environment
  2. Therapeutic practice
  3. Education of PT/ OT students, other health professionals and/or the public
  4. Administration
  5. Political influence and advocacy
  6. Other

Nomination Process

To nominate an alumnus for this prestigious award, please complete the nomination form found below, assemble the required package of information in cooperation with the nominee, and mail or email all information to the PT/OS&OT Alumni Association Office at:

Required Package of Nomination Information

Nominations should demonstrate significant contributions in three or more of the six categories outlined on pages one and two and must include:

  • A completed Nomination Form detailing the nominee’s professional achievements and contributions in each of at least 3 categories on the nomination form 
  • Nominee’s current Curriculum Vitae or Resume

The Selection Committee’s decisions will be based on a review of the candidate’s contributions as described in the submission. Nom­inators are strongly encouraged to provide detailed information on their nominees.
