100th Anniversary “Notable Physiotherapy Alumni” Nomination Form
As part of the 100th anniversary of Physical Therapy at the University of Toronto, we are recognizing notable physiotherapy graduates. These are outstanding physiotherapists that have made a significant impact in related practice, education, research, advocacy or in the public domain. Selected notable graduates will be featured on our website and through other communications channels including Twitter and PT Link Newsletter.
We are looking for your help! Please consider nominating a notable graduate. Nominees should meet all the following criteria:
- Evidence of outstanding accomplishment and leadership towards the advancement of related practice and/or education and/or research and/or public service.
- Graduate from the U of T physical therapy or combined PT/OT programs with a professional degree/designation.
- You must have approval from the person you are nominating or their family (for nomination posthumously).
Please complete this very brief nomination form. Note that you will be required to provide a citation of 2000 characters or less describing the accomplishments and contributions of the nominee. Although not required, there will be an opportunity to upload a photo of the nominee.
