“Optimizing transitions in care for populations with complex needs: Implications for rehabilitation”
Presenter: Dr. Sara Guilcher
Dr. Sara Guilcher (PT, PhD) is an Assistant Professor at the Leslie Dan Faculty of Pharmacy and has cross-appointments to the Institute for Health Policy, Management and Evaluation and the Rehabilitation Sciences Institute at the University of Toronto, Ontario, Canada. As a physical therapist funded by the Canadian Institutes for Health Research (CIHR) Embedded Clinician Scientist program, Dr. Sara Guilcher leverages the intersection of rehabilitation sciences, implementation sciences with learning health systems. Working with Health Quality Ontario, Dr. Sara Guilcher is creating innovative science that optimizes integration of care for whole-person activation among individuals with complex needs. Dr. Sara Guilcher has expertise in transitions in care, alternate level of care, system integration, self-management, multimorbidity, disability, equity, evaluation methods, and mixed-methods.
